School Age Program Rates
August 2008 - June 2009 (Effective August 27th, 2008)

Registration Fee: $40.00 per child, annually (nonrefundable)

Sibling discount of 10% for each additional child
Excludes siblings in the 4 year old preschool and additional charges to your monthly tuition)

REGISTRATION FORMS
for Before & After School Care:
The following forms and information are required by the Department of Human Resources (D.H.R.) for any child admitted into care. All forms must be completed, signed and returned to the center Director 1 week prior to your child's start date with a non refundable fee of $50.00 per family, made out to the Sunshine Kids' Club. We cannot make any exceptions. If forms are found to be incomplete your child cannot attend the center. If you have any questions please feel free to call the center during operating hours at (410) 549-0844.

(2) Emergency Cards - (completed and signed) Health Inventory - (4 pages) Maryland Immunization Certificate Lead Testing Certificate All About My "Child" Pick-up Authorization Form Consumer Education Brochure entitled "Parent's Guide to Regulated Child Care" (this needs to be signed)

It is the Parents responsibility to update their child's emergency cards every 6 months to keep all vital information current for the general welfare and safety of their child(ren).
