Dear Parents:
In addition to a completed registration form for the fall, we are required by the state of Maryland to have the following information of all children in our centers on file. Within the files, we MUST have the following:

Lead Testing Certificate

"All About_________" Form

"A Parent's Guide To Regulated Child Care"

Publicity Form Release (not required - will be posted)

Please Click each link above to print and fill out each form to be turned in prior to the first day, or bring them with your child on the first day of attendance. State Law mandates: we may not permit students to attend without these forms. NO EXCEPTIONS.

Again..... These forms must be completed and returned to the center before or on the first day of attendance.
